Prize Drawing for Complying with Preventive Care and Disease Management

In addition, we will conduct a prize drawing in September 2012 for participants who are covered for health and (1) qualify for a $75 gift card (complete the Health Risk Appraisal discussed above);  (2) get a routine physical, and (3) enroll in a disease management program (if eligible) by June 30, 2012. Prizes will consist of:

    4   $250 gift cards
