- 1. Go to www.CSIonline.org/benefits and under U.S. Insurance (left hand side of page) select Links. Select the link titled “Trustmark“, which is the link to Trustmark’s Healthy Foundations website.
- 2. Once in the Trustmark web site, under the HealthCenter Section (near the bottom of the page) click on click here next to where it says “Log on to the HealthCenter.“
- 3. New users should select Register Here, complete the registration, and click on Submit Form.
Existing users should enter their User Id and Password and click on Login. Any employee who did not complete a Trustmark Health Risk Appraisal in the last year will need to complete the WorldDoc Registration. Employees who did complete a Health Risk Appraisal last year should select either “Yes, you may contact me” or “No, I do not wish to be contacted” to receive e-mail notifications on how to improve their health. Then click on “Enter the HealthCenter.“
- 4. Employees who did not complete a Trustmark Health Risk Appraisal in the last year will need to complete the Security Questions, enter the required Contact Information and check to agree to the Privacy Policy and Terms and Conditions. Then click on Continue.
- 5. Once in the HealthCenter, click on the “use it” link on the Health Risk Assessment Icon at the upper right of the page.
- 6. On the page that comes up, the employee’s name should be listed next to “Take this Health Assessment for:“ If not, click on the box to select the employee’s name.“
- 7. Select “Click here to take the assessment"at the bottom of the page.
- 8. Complete the first section of the Health Risk Appraisal and click on “Save and go to next step” at the bottom of the page.
- 9. Complete the second section of the Health Risk Appraisal and click on “Save and go to next step” at the bottom of the page.
- 10. Complete the final section of the Health Risk Appraisal and click on “Save and see your results” at the bottom of the page.
