If you have questions regarding specific claims, contact Sun Life Customer Service at 800-361-6212. When calling Sun Life, you will need the CSI policy number (14284) and your social insurance number (both are on your insurance card). If you need to submit a claim, forms can be printed from the CSI web site. Go to http://www.CSIonline.org and select Employee Benefits, Canada, CSI Canada Insurance Plan, and Forms.
