No Evidence Requirement for Transfer from Limited Benefit to Flex

Under the current plan arrangement, there are certain restrictions for those in the Limited Benefit Plan (Life, AD&D, and Disability) who wish to opt back into the health and dental plan.  Historically, the Plan has required evidence of good health and limited the first year dental coverage to $250.  Effective September 1 2011, these two limitations are being eliminated.  Going forward, participants can switch from the Limited Benefit Plan to a plan with health and dental coverage:
