Coverage Options following Termination of CSI Insurance

CSI continues to offer a program for life, health and dental insurance coverage to employees and dependants who are no longer covered by the CSI Canada Insurance Plan. For health and dental insurance, one option is Manulife Financial’s FollowMe health and dental plan which is designed for employees who leave their school before retirement (for example, due to career change or job loss) and are not eligible for the CSI Retiree Insurance Plan and dependants who no longer are qualified to be on the Plan. 
This benefit transfers participants from a group to personal health care plan without a need to submit medical information, provided the application is made within 60 days of leaving the CSI Canada Insurance Plan (guaranteed acceptance dependent upon receipt of first premium payment). Applications submitted after the 60 day period will require full medical underwriting. It is affordable coverage for unexpected health problems and emergencies, as well as many on-going health-related expenses not covered by provincial plans. The benefits do not mirror the CSI-Canada Insurance Plan.

Another option for continued health and dental coverage that requires you to submit medical information is Manulife’s FlexCare plan.  This is a flexible and comprehensive health insurance plan that allows you to custom-design your health insurance coverage based on you and your family’s needs.  This option is best suited for those in good health.  For additional information about this plan contact Manulife Financial’s Customer Service Representatives at 1-877-COVERME (1-877-268-6195) or log on to http://www.coverme.com.
There are two programs available to continue your Life insurance coverage.  One program is CSI’s Group Life Conversion plan which has no coverage restrictions and allows you to convert up to $200,000 in life insurance to an individual plan without the need to submit medical evidence providing you submit your application and first month’s premium within 31 days of losing your group life coverage.  Contact Manulife Financial at 1-800-268-6195 for more information on this option or log on to http://www.coverme.com., phone number 1-877-COVERME (1-877-268-3763).
The second program is called FollowMe Life that allows you and/or your spouse to convert up to $200,000 each of your group coverage without the need to submit medical evidence providing the application and premiums are sent in within 60 days of leaving the CSI Canada Insurance Plan.  This plan has very affordable rates and a restriction with this plan is that coverage will not be extended to persons who are ill or insured at the time of application.  Also, benefits will not be paid where death results from suicide within two years of the effective date of coverage.  This option is bested suited for individuals in good health.  For more information, log on to http://www.coverme.com or contact Manulife Financial’s Customer Service Representatives at 1-877-COVERME (1-877-268-3763).
